Ask HN: Software/system for official notes/records

Does anyone know, what kinds of software/systems/practices do organizations use for notes, diary entries, records, by executives, leaders, CEOs, etc? Just folders of MS Turd documents? Or something else?

Increasingly SharePoint, in my experience contracting at diverse organisations over the past decade or so. Automatic version control, all the access control you could want, and you can chuck any old file in it (though obviously it works best with MS Office formats, which you can view and edit in a browser to some extent. ‘CVS for normals’, if you like, but more besides. Corporates love it because they trust Microsoft.

Computer based records could be controlled via CVS (Concurrent Versions System) if available from the OS.
